Eficacia de la hipoterapia en el tratamiento de la espasticidad de los niños con daño cerebral

Abstract

Este trabajo de fin de grado trata del diseño teórico de un estudio experimental, con dos grupos aleatorios, un grupo control y un grupo intervención. Su objetivo principal es de valorar si el tratamiento de hipoterapia tiene efectos positivos a nivel de la espasticidad de los niños con daño cerebral. Entonces se seleccionarán niños que siguen sus tratamientos de fisioterapia en el centro “Autour de l’enfant” de Montpellier en Francia a través de los criterios de inclusión y de exclusión planteados. Estos niños serán divididos de manera aleatoria en 2 grupos iguales compuestos 23 individuos en cada uno. Un grupo será definido como el grupo control que seguirá solamente el tratamiento multidisciplinario y de fisioterapia mientras que el grupo intervención seguirá en más de estos tratamientos sesiones de hipoterapia una vez a la semana durante 6 meses. Durante el estudio se realizarán 3 momentos de valoración idénticas en los dos grupos es decir con los mismos tests y en el mismo tiempo. El primero se realizará antes de empezar el estudio, el segundo al medio del estudio es decir a los 3 meses, y el último al final de los 6 meses. En cada momento de valoración se valorará la espasticidad a través de la escala de Ashworth, la calidad de vida a través del KINDL y el estrés a través del Youth Outcomes Questionnarie. La espasticidad será valorada por un fisioterapeuta y los cuestionarios de calidad de vida y del estrés serán rellanados por los padres y los niños. Después gracias al programa estadístico SPSS Statistics se analizarán los datos recogidos a través de las medias, deviaciones estándares y de los porcentajes. Desde aquí se propasarán conclusiones para poner en evidencia si existen efectos positivos de la hipoterapia sobre los niños con daño cerebral. Por otra parte, dentro de este estudio se encontraron varias limitaciones. Efectivamente muchos parámetros pueden interferir en los resultados y sobre todo en las conclusiones. Es decir que en el estudio existen muchos factores que crean una heterogeneidad entre los niños como la clínica que está muy variada de un niño al otro, la hipoterapia también cambia en función del caballo que se utiliza. También como la hipoterapia se realiza en el exterior la buena realización de todas las sesiones depende del tiempo. Además la hipoterapia es una terapia cara que no está pagada por la seguridad social o mutua, y pide disponibilidad de los padres para llevar el niño todas las semanas. Por eso será difícil de encontrar un número suficiente de niños para realizar este estudio.

This master piece takes place in the achievement of my studies. It has been designed to deal with an experimental study among two random groups : a control and an intervention one. The main goal of this study is assessing whether the hypotherapy treatment has positive effects and consequences on childrens with cerebral palsy. Will be selected childrens with a kinesitherapy treatment in the health care center named « autour de l’enfant » based in Montpellier. Those childrens have been selected with two inclusion and exclusion criteria set up in advance. They will be randomly divided into two similar groups composed by 23 individuals each. One group will be the control group. Childrens from this group will receive a multidisciplinary treatment of kinesitherapy. The other one, the intervention one will receive the same treatment plus hypotherapy sessions once a week for 6 months. Along this whole study, 3 assessment periods will be operated among those two groups : at the same time and in the same conditions. The first one will occur before the beginning of the hypotherapy sessions among the intervention group, the second after 3 months and the third and last one at the end of the 6 months. At each of these moments, will be evaluated : - The spasticity thourgh Ashworth’s scale - The way of living thanks to KINDL test - The stress through the Youth Outcomes survey The spasticity will be assessed by physiotherapists and the surveys about the quality of living and the stress will be filled out directly by the parents themselves. Then, thanks to the SPSS Statistics program, the results will be analyzed through averages, standards and pourcentages. According to those results, conculsions will be given for underlining the effects of hypotherapy on the childrens with cerebral palsy. In an other hand, it’s important to mention that many hindrances can potentially affect the mainstream of our study : many factors can have consequences on our results, impacting directly our conclusions. In other words, some factors can create an hetereogenity among the childrens. That’s for instance the case when we talk about the health care center, from one children to another, the results of the hypotherapy can highly depends on the horse itself... As the hyptherapy is an outdoor method, our sessions will depend on the weather. Furthermore, the hypotherapy is an expensive treatment method and the social security or health care funds do not cover its cost. It also requires availability among parents who decide using this method. All these reasons lead to a lack of the number of childrens for achieving correctly this study.
