Análisis de la refractariedad del malestar emocional en pacientes paliativos pediátricos: Metodología Delphi

Abstract

Cicely Saunders reconoció en el sufrimiento, el objetivo central de los cuidados paliativos. El sufrimiento al final de la vida presenta varias esferas, siendo el área emocional una de ellas. El personal que trabaja en unidades de cuidados paliativos ha seguido una formación experiencial y teórica en éste área. Sin embargo, a pesar de la bondad de la atención psicológica, en algunas ocasiones el malestar producido por síntomas emocionales, así como la interacción con otros síntomas de naturaleza diversa no logra disminuirse. Por añadidura, los niños y adolescentes no están exentos de estos síntomas. Sin embargo, la evidencia en este aspecto es aún más escasa, no respondiendo a las necesidades reales de los profesionales que ejercen su labor en servicios de cuidados paliativos pediátricos. Objetivo: Analizar el malestar emocional en pacientes paliativos pediátricos y su refractariedad. Método: Estudio Delphi con panel de expertos constituido por profesionales de cuidados paliativos, el contenido se centrará en la definición del malestar emocional, factores incidentes y criterios diagnósticos, evaluación, tratamiento y criterios de refractariedad. El presente proyecto pretende unificar terminologías y criterios, así como facilitar la identificación, evaluación y abordaje del malestar emocional en pacientes paliativos pediátricos.

Cicely Saunders recognized suffering as the main objective of palliative care. Suffering at the end of life presents various spheres, the emotional sphere being one of them. Personnel who work in palliative care have followed practical and theoretical training. However, despite the good intentions of psychological care, at times the distress produced by emotional symptoms and which also interacts with other symptoms cannot be lowered. It also must also be said that children and adolescents are not exempt from these symptoms. However, there is even less evidence of this aspect and it doesn’t respond to the real needs of the professionals that work in pediatric palliative care. Objective: To analyze emotional distress in pediatric palliative patients and the possibility of its becoming refractory. Method: Delphi Method with a panel of experts made up of professional palliative care givers. The content will be focused on the definition of emotional distress, incidental factors and diagnostic criteria, evaluation, treatment and refractory criteria. The present Project intends to unify terminology and criteria as well as to facilitate the identification, evaluation and the approach to emotional unease in pediatric palliative patients.
