Efectivitat de la teràpia de moviment induït per restricció modificada en infants amb paràlisis cerebral

Abstract

El present estudi és un assaig clínic aleatoritzat, longitudinal, quasi experimental i de caràcter prospectiu. El seu objectiu és demostrar l'efectivitat de la teràpia de moviment induït per restricció en preescolars amb paràlisis cerebral espàstica. Es seleccionaran a 156 nens d'entre 4 i 6 anys amb un nivell 2 o inferior en l'escala d'Ashworth modificada i un nivell 3 o inferior de l'escala GMFCS. Aquests infants es dividiran en dos grups de manera aleatòria. El grup experimental rebrà tractament de fisioteràpia convencional mitjançant el mètode Bobath i a continuació realitzarà uns exercicis específics d'entrenament de la motricitat fina. Per altra banda, el grup control realitzarà el mateix tractament de fisioteràpia convencional i a més a més també farà els exercicis específics d'entrenament per millorar la funcionalitat manual però seguint el mètode de restricció de moviment de l'extremitat sana. Per tal de saber si la teràpia de moviment induït per restricció modificada és efectiva es mesurarà l'espasticitat mitjançant l'Escala d'Ashword modificada i s'aplicarà el test MACS per valorar l'habilitat manual. Els testos es realitzaran 4 vegades durant tot l'estudi, per tal de valorar la millora i si aquesta es manté en el temps. Al final de l'estudi, també es passarà un qüestionari de satisfacció als pares per tal de valorar la seva opinió respecte al tractament i l'atenció rebuda. Limitacions: Dificultat per calcular la mostra, l'estudi pot augmentar la seva durada en el temps depenent de si s'aconsegueix la mostra en el mateix moment, el rebuig de la fèrula o la frustració que aquesta pot provocar, les possibles baixes dels subjectes o dels professionals i la falta validesa científica de les eines de mesura.

This is a randomized, longitudinal, almost experimental and prospective clinical trial. Its aim is to show the effect of the constraint-induced movement therapy (CIMT) in pre-schoolers with spastic cerebral palsy. 156 children between 4 and 6 years old, with a level 2 or inferior in the Modified Ashworth Scale and with a level 3 or inferior in the GMFCS Scale will be selected and randomly divided in two research groups: an experimental group and a control group. On one hand, the experimental group will receive conventional physiotherapy treatment using the Bobath method, and later on will carry out specific exercises to improve their fine motor skills. On the other hand, the control group will follow the same conventional physiotherapy treatment, and will later undergo specific exercises to improve the motor function of the upper extremities following the constraint-induced movement therapy method. In order to find out whether the constraint-induced movement therapy is effective, the spasticity will be measured using the Modified Ashworth Scale and the MACS test will be applied to identify the manual ability. The tests will be carried out four times during the study. A satisfaction questionnaire will be given to the parents in order to value their opinion on the treatment and the attention received. Limitations: Possible problems calculating the number of subjects that can undertake the study; the study may take more time than estimated depending on availability of the subjects; rejection or frustration that the splint may cause; possible sick leaves of the subjects or the professionals; the lack of scientific evidence of the evaluating tools.
