Creació d’un biobanc de sèrums de pacients amb ferides cròniques cutànies

Abstract

Les ferides cròniques són un problema assistencial greu i en augment en els països desenvolupats. Una de les fallades del procés fisiològic de curació d’una ferida és la deficient irrigació de la zona danyada que impedeix l'arribada dels factors moleculars necessaris perquè les cèl·lules n’iniciïn la curació. La teràpia avançada amb plasma ric en plaquetes autòleg (PRP) facilita l'arribada de factors del propi pacient al llit de la ferida, cosa que activa les cèl·lules viables del propi teixit i n’estimula la cicatrització. L’objectiu principal d’aquest treball és establir els protocols de recol·lecció, processat i emmagatzematge de mostres de sèrum de pacients sotmesos a una teràpia amb PRP per al tractament de ferides cròniques cutànies seguint les mesures de qualitat i bioseguretat oportunes. Per a l’assoliment d’aquest objectiu, s’han hagut de conèixer les normatives vigents i aspectes ètics que regulen la investigació i la recollida de mostres humanes, s’ha hagut de dissenyar i posar en marxa un laboratori de criopreservació de mostres que servís com a biobanc, s’han hagut de redactar els procediments normalitzats de treball (PNT) d’ús dels equipaments adquirits pel projecte i s’han hagut d’elaborar els protocols de transport i manipulació de les mostres complint les pautes de bioseguretat recomanades. En conclusió, s’han buscat i interpretat les normatives que són d’aplicació en el projecte, i s’han adaptat i implantat en cadascun dels contextos del treball. A més, s’han elaborat els protocols necessaris per a la posada en marxa d’un projecte d’estudi dels mecanismes d’acció de la teràpia amb PRP.

Chronic wounds are a serious and growing healthcare problem in developed countries. One of the shortcomings of the physiological process of wound healing is the poor irrigation of the damaged area that prevents the arrival of the molecular factors necessary for the cells to begin healing. The autologous platelet-rich plasma (PRP) advanced therapy facilitates the patient’s own factors arrival to the wound bed, activating viable tissue cells and stimulating healing. The main objective of this work is to establish the protocols for the collection, processing and storage of serum samples from patients undergoing PRP therapy for the treatment of chronic cutaneous wounds following appropriate quality and biosecurity measures. To achieve this goal, it has been necessary to know the current regulations and ethical aspects that regulate the research and collection of human samples, it has been necessary to design and set up a samples cryopreservation laboratory to serve as a “biobank”, the standard operating procedures (SOP) for the use of the equipment acquired by the project had to be drafted and the protocols for transporting and handling the samples had to be drawn up in compliance with the recommended biosecurity guidelines. In conclusion, the regulations that apply to the project have been sought and interpreted and have been adapted and implemented in each of the contexts of the work. In addition, the necessary protocols have been developed for the implementation of a project to study the mechanisms of action of PRP therapy.
