Efectivitat de la fisioteràpia preoperativa en la rehabilitació dels nens post rotationplasty d'un osteosarcoma de l'extremitat inferior

Abstract

L'objectiu d'aquest estudi és avaluar l'efecte d'una intervenció de fisioteràpia preoperatòria sobre els paràmetres funcionals, la qualitat de vida, la força muscular, l'amplitud articular i l'equilibri després d'una Rotationplasty. Es tracta d’un assaig clínic aleatoritzat controlat, simple cec. 10 pacients (entre 8 i 15 anys) als quals se'ls va programar una Rotationplasty en l'Institut Gustave Roussy per un osteosarcoma de l'extremitat inferior a partir de setembre de 2022. Els participants seran assignats aleatòriament al Grup de Control (GC) o al Grup Experimental (GE) per una durada de seguiment de 41 setmanes. 6 setmanes abans de la cirurgia, el GE realitzarà 18 sessions de fisioteràpia preoperatòria que conté exercicis de resistència cardiovascular, estirament muscular, reforç muscular, exercicis d'equilibri i mobilització activa de les articulacions de les extremitats inferiors. Les 4 mesures es faran per a cada grup abans, després, 4 mesos després de l'operació i una al final de la intervenció. Les escales de mesura van ser la pediatric Toronto Extremity Salvage Score (pTESS), la Musculoskeletal Tumor Society Scale (MSTS), Pediatric Quality of Life (PedsQL™), Medical Research Council Manual Testing Scale (MRC), l'amplitud articular del turmell (ROM) i la Pediatric Balance Scale (PBS). L'anàlisi de les dades recollides es durà a terme amb el programa informàtic IBM® SPSS statistics 28.0.1. Les limitacions que podríem trobar estan relacionades amb la mida de la mostra a causa del nombre de participants reduït i la seva dificultat per a reclutar-los. El fet de tenir les escales pTESS i MSTS no validades a Espanya pot ser una limitació.

The purpose of this study was to assess the effect of a preoperative physical therapy intervention on functional parameter, quality of life, muscle strength, joint range of motion and balance after Rotationplasty. This is a randomized controlled trial, simple blind. 10 patients (age range 8-15 years) scheduled for Rotationplasty at the Gustave Roussy Institute for osteosarcoma of the lower extremity from September 2022. Participants will be randomly assigned to the Control Group (CG) or Experimental Group (EG) for a follow-up period of 41 weeks. 6 weeks prior to surgery, the EG will perform 18 sessions of preoperative physical therapy containing cardiovascular resistance exercises, muscle stretching, muscle strengthening, balance exercises and active mobilisation of the lower extremity joints. The 4 measurements will be made for each group before the operation, immediately after the operation, 4 months after the operation and at the end of the operation. The measurement variables were the pediatric Toronto Extremity Salvage Score (pTESS), Musculoskeletal Tumor Society Scale (MSTS), Pediatric Quality of Life (PedsQL™), Medical Research Council Manual Testing Scale (MRC), ankle range of motion (ROM) and the Pediatric Balance Scale (PBS). The analysis of the collected data will be done with the IBM® SPSS statistics 28.0.1 software. The limitations that we might find are related to the size of the sample due to the small number of participants and the difficulty of recruiting participants. The fact that the pTESS and MSTS scales are not validated in Spain may be a limitation.
