Impacte d'un checklist quirúrgic modificat sobre el control de les infeccions de la localitat quirúrgica

Abstract

Introducció: Els professionals d’infermeria, tenen la responsabilitat de supervisar i comprovar periòdicament els diversos processos administratius i operatius de l’aplicació rigorosa de les normes i els principis d’asèpsia i antisèpsia quirúrgica. Per això, és de vital importància l’aplicació d’aquestes eines amb la finalitat d’evitar que les infeccions de localitat quirúrgica (ILQ) relacionades amb l’assistència sanitària es produeixin. Actualment, el cheklist quirúrgic és un dels instruments fonamentals que s’utilitzen per prevenir aquestes infeccions. L’ús correcte del checklist quirúrgic reforça la pràctica segura i alhora garanteix la seguretat dels pacients a quiròfan. Objectiu: Avaluar l’impacte d’un checklist quirúrgic modificat tenint en compte ítems de baix risc marcats pel projecte PrevinQ-cat l’any 2023 i altres estudis relacionats amb la prevenció d’ILQ sobre la millora de l’acompliment del rol infermer, el control i detecció dels riscos en l’àrea quirúrgica i la taxa de les ILQ i la seva severitat, respecte al checklist habitual. Metodologia: Es portarà a terme un assaig clínic aleatoritzat en dos grups: Grup Control (on es portarà a terme el cheecklist quirúrgic que s’utilitza habitualment) i Grup Intervenció (on es portarà a terme el cheecklist modificat segons els ítems del projecte PrevinQ-cat i altres estudis relacionats amb la prevenció d’ILQ) a l’àrea quirúrgica de l’Hospital Universitari de Vic, l’any 2024. La població d’estudi seran els pacients intervinguts de manera programada vinculats amb el checklist i les infermeres quirúrgiques que el complimenten. Variables d’estudi relacionades amb els pacients: Sociodemogràfiques, clíniques i percentatge d’infeccions. Variables d’estudi relacionades amb les infermeres: Variables sociodemogràfiques, L’acompliment del rol (indicadors NOC) i control i detecció dels riscos (indicadors NOC). La recollida de dades tindrà lloc a l’inici de l’estudi i al cap de 10 mesos. Limitacions: Dificultat per trobar pacients i infermeres que puguin complir els criteris d'inclusió i exclusió i estiguin disposats a formar part de l'estudi, complint els requisits demanats. Falta de compromís per part de les infermeres. El Grup Control ho relacioni de forma negativa, sabent que hi ha un grup beneficiat per rebre una intervenció addicional. Introduction: Nursing professionals have the responsibility to supervise and periodically check the various administrative and operational processes of the rigorous application of the rules and principles of asepsis and surgical antisepsis. For this reason, the application of these tools is vitally important in order to prevent surgical site infections (SIIs) related to health care from occurring. Currently, the surgical checklist is one of the fundamental instruments used to prevent these infections. The correct use of the surgical checklist reinforces safe practice and at the same time guarantees the safety of patients in the operating room. Objective: To evaluate the impact of a modified surgical checklist taking into account low-risk items marked by the PrevinQ-cat project in 2023 and other studies related to the prevention of surgical site infections, on the improvement of role performance nurse, the control and detection of risks in the surgical area and the rate of these infections and their severity, with respect to the usual checklist. Methodology: A randomized clinical trial will be carried out in two groups: Control Group (where the commonly used surgical checklist will be carried out) and Intervention Group (where the modified checklist will be carried out according to the items of the PrevinQ-cat project and other studies related to the prevention of surgical site infections ) in the surgical area of the Hospital Universitari de Vic, in the year 2024. The study population will be the patients intervened in a programmed manner linked to the checklist and the surgical nurses who they comply Study variables related to the patients: Sociodemographics, clinics and percentage of infections. Study variables related to nurses: Socio-demographic variables, Role performance (NOC indicators) and risk control and detection (NOC indicators). Data collection will take place at the beginning of the study and after 10 months. Limitations: Difficulty in finding patients and nurses who can meet the inclusion and exclusion criteria and are willing to be part of the study, fulfilling the requested requirements.
